To begin services: 

1. Contact your Support Coordinator or Planning List Administrator and request they open your records for the Chanan Foundation to do a review. 
2. If we determine that we can meet your service needs, we will schedule a tour 3. If you choose Chanan as your provider, we will begin the intake process.

Services are provided throughout our program locations and in community settings such as: 

• Parks 
• Restaurants 
• Volunteer locations
• Local businesses 
• Community events 

We believe community integration is a vital part of personal development.

Yes. Transportation may be available for participants who utilize services on a consistent schedule and fall within a 12 mile radius of either program location. 

Families are also welcome to provide transportation if they prefer.

Activities vary on skill level but may include: 

• Community outings 
• Fitness and recreational activities 
• Life skills development 
• Volunteer opportunities 
• Social events 
• Educational workshops 
• Art and Crafts 

Our goal is to provide engaging, purposeful experiences that build confidence and independence.

Participants typically bring their own lunch unless otherwise noted for special outings or events. Staff will communicate in advance if food will be provided or purchased during a community activity.

Most participants receive services through the NOW or COMP Medicaid waiver. These waivers cover approved services based on the participant’s Individual Service Plan.

Families receive regular communication through: 

• Program calendars 
• Chanan Foundation App 
• Special event announcements 

Annual Individual Service Plan (ISP) meetings will be held to discuss progress and make plans for the next year's services and individual goals. 

You will also receive the director's phone number for important communication needs. We value strong communication and partnership with families.

All of our homes are licensed for 3 to 4 individuals. We provide round the clock staffing when individuals are in their homes. We typically have a waiting list for these services. If you are interested, reach out today.

If someone has CLS on their waiver, we can provide private housing with staffing assistance in accordance with the hours noted in the ISP. We typically have a waiting list for these services. If you are interested, reach out today.

Absolutely! We love when families are involved and come to visit. You are also welcome to take your loved one home for a visit. 

A Host Home is a residential service model where an individual with an intellectual or developmental disability lives in a private family home with a trained caregiver (the host) rather than in a traditional group home. These homes are contracted and trained through The Chanan Foundation.
